    Mountain bike racing - the influence of prior glycogen-inducing exercise and glutamine supplementation on selected stress and immune parameters

    Objective. To investigate the effect of pre-exercise glutamine supplementation and the influence of a prior acute bout of glycogen-reducing exercise on the general stress and immune response to acute high-intensity cycling. Design. Randomised, double-blind, cross-over supplementation study. Setting and intervention. Subjects performed a series of 4 simulated mountain-bike races lasting &#8776;60 minutes each on separate days 1 week apart, with/ without prior glycogen- reducing exercise on a known outdoor course with/ without pre-exercise glutamine supplementation. Blood samples were collected pre- and immediately post-exercise after each race. Main outcome measures. Circulating concentrations of cortisol (COR) and dehydroepiandrosterone-sulphate (DHEAS) were assessed at all time points, as well as changes in white blood cell (WBC) subpopulation distribution. Results. COR was elevated in all groups post-exercise (p < 0.0001), but neither glycogen reduction, nor glutamine supplementation had any effect. DHEAs increased post-exercise (p < 0.05), with a greater relative increase in glutamine-supplemented subjects (p = 0.07). Total WBC and neutrophil counts in all groups were elevated after exercise (both p < 0.0005). Glutamine supplementation had no effect on differential WBC counts or distribution, but total WBC (p = 0.06) and monocyte (p < 0.05) counts showed greater increases after glycogen reduction. Gluta- mine supplementation was associated with greater postexercise decreases in CD4+ count (p = 0.07) and CD4+: CD8+ ratio (p = 0.01) after glycogen-reducing exercise. Conclusions. We conclude that pre-exercise glutamine supplementation may have an anticortisol effect by enhancing the DHEAS response to exercise stress. The suppressive effect of glutamine supplementation on CD4+: CD8+ ratio and its positive effect on monocyte count after repeated bouts of exercise warrants further investigation.     Acute change of titin at mid-sarcomere remains despite 8 wk of plyometric training.

    Purpose: The purpose of this study was to investigate skeletal muscle changes induced by an acute bout of plyometric exercise (PlyEx) both before and after PlyEx training, to understand if titin is affected differently after PlyEx training. Methods: Healthy untrained individuals (N=11) completed the 1stPlyEx (10x10 squat-jumps, 1min rest). Thereafter, 6 subjects completed 8 weeks of PlyEx, while 5 controls abstained from any jumping activity. Seven days after the last training session all subjects completed the 2ndPlyEx. Blood samples were collected before, 6 hours and 1, 2, 3 and 4 days after each acute bout of PlyEx, and muscle biopsies 4 days before and 3 days after each acute bout of PlyEx. Results: The 1stPlyEx induced an increase in circulating myoglobin concentration. Muscle sample analysis revealed Z-disk streaming, a stretch or a fragmentation of titin (immunogold), and increased calpain-3 autolysis. After training, 2ndPlyEx did not induce Z-disk streaming, or calpain-3 activation. The previously observed post-1stPlyEx positional change of the titin c-terminus was still present pre-2ndPlyEx, in all trained and all control subjects. Only 2 controls presented with Z-disk streaming after 2ndPlyEx, while calpain-3 activation was absent in all controls. Discussion: Eccentric explosive exercise induced a stretch or fragmentation of titin, which presented as a positional change of the c-terminus. Calpain-3 activation does not occur when titin is already stretched before explosive jumping. Enzymatic digestion results in titin fragmentation, but since an increase in calpain-3 autolysis was visible only after the 1stPlyEx acute bout, fragmentation cannot explain the prolonged positional change

    The effect of a professional cricket match schedule on selected immune parameters

    Objective. The impact of a professional cricket match schedule on white blood cell (WBC) distribution and lymphocyte activation (CD69 expression) was investigated. Methods. After a 3-month pre-season training period, physical and immune parameters were determined in 14 male cricketers before (B) and after (A) an intensive 5- week match schedule. Results. Exercise test results were unchanged from B to A. Total WBC counts were similar, but total lymphocyte and lymphocyte subpopulation counts decreased significantly. The CD4:CD8 ratio did not change. After in vitro stimulation, percentage CD4+CD69+ cells increased (B: 54.4 ± 9.7%, A: 64.0 ± 8.5%, p < 0.01), but absolute CD4+CD69+ cell counts did not change from B to A. In contrast, both the %CD8+CD69+ cells and absolute CD8+CD69+ cell count remained similar. Conclusion. A strenuous, interregional, professional cricket match schedule resulted in a decreased number of lymphocytes, but relatively increased in vitro reactivity of CD4+ cells, thus maintaining the absolute capacity of the CD4+ cells to become activated on stimulation. In cricketers who suffered upper respiratory tract symptoms during the match schedule (N = 7), none of the immune parameters investigated differed significantly from the others at B or A.     Leadership and capacity building in international chiropractic research: introducing the chiropractic academy for research leadership (CARL).

    In an evidence-based health care environment, healthcare professions require a sustainable research culture to remain relevant. At present however, there is not a mature research culture across the chiropractic profession largely due to deficiencies in research capacity and leadership, which may be caused by a lack of chiropractic teaching programs in major universities. As a response to this challenge the Chiropractic Academy for Research Leadership, CARL, was created with the aim of develop a global network of successful early-career chiropractic researchers under the mentorship of three successful senior academics from Australia, Canada, and Denmark. The program centres upon an annual week-long program residential that rotates continental locations over the first three-year cycle and between residentials the CARL fellows work on self-initiated research and leadership initiatives. Through a competivite application process, the first cohort was selected and consists of 13 early career researchers from five professions in seven countries who represent diverse areas of interests of high relevance for chiropractic. The first residential was held in Odense, Denmark, with the second being planned in April 2018 in Edmonton, Canada, and the final residential to be held in Sydney, Australia in 2019

    Effects of an exercise programme with people living with HIV: Research in a disadvantaged setting

    This study aimed to analyse the physical health effects of a community based 10-week physical activity programme with people living with HIV. It was developed, implemented and evaluated in a disadvantaged community in South Africa. A pre-post research design was chosen. Major recruitment and adherence challenges resulted in a small sample. Among the 23 participants who took part in both baseline and final testing, compliant participants (n = 12) were compared to non-compliant participants (n = 11). Immunological (CD4, viral load), anthropometric (height, weight, skinfolds and waist to hip ratio), muscular strength (h1RM) and cardiopulmonary fitness (time on treadmill) parameters were measured. The compliant and non-compliant groups were not different at baseline. Muscular strength was the parameter most influenced by compliance with the physical activity programme (F = 4.516, p = 0.047). Weight loss and improvement in cardiopulmonary fitness were restricted by the duration of the programme, compliance and influencing factors (e.g. nutrition, medication). The increase in strength is significant and meaningful in the context, as the participants goals were to look healthy and strong to avoid HIV related stigma. The improvements in appearance were a motivational factor, especially since the changes were made visible in a short time. Practical implications for health promotion are described. More research contextualised in disadvantaged settings is needed.DHE

    Aerobic capacity, activity levels and daily energy expenditure in male and female adolescents of the kenyan nandi sub-group

    The relative importance of genetic and socio-cultural influences contributing to the success of east Africans in endurance athletics remains unknown in part because the pre-training phenotype of this population remains incompletely assessed. Here cardiopulmonary fitness, physical activity levels, distance travelled to school and daily energy expenditure in 15 habitually active male (13.9±1.6 years) and 15 habitually active female (13.9±1.2) adolescents from a rural Nandi primary school are assessed. Aerobic capacity ([Formula: see text]) was evaluated during two maximal discontinuous incremental exercise tests; physical activity using accelerometry combined with a global positioning system; and energy expenditure using the doubly labelled water method. The [Formula: see text] of the male and female adolescents were 73.9±5.7 ml(.) kg(-1.) min(-1) and 61.5±6.3 ml(.) kg(-1.) min(-1), respectively. Total time spent in sedentary, light, moderate and vigorous physical activities per day was 406±63 min (50% of total monitored time), 244±56 min (30%), 75±18 min (9%) and 82±30 min (10%). Average total daily distance travelled to and from school was 7.5±3.0 km (0.8-13.4 km). Mean daily energy expenditure, activity-induced energy expenditure and physical activity level was 12.2±3.4 MJ(.) day(-1), 5.4±3.0 MJ(.) day(-1) and 2.2±0.6. 70.6% of the variation in [Formula: see text] was explained by sex (partial R(2) = 54.7%) and body mass index (partial R(2) = 15.9%). Energy expenditure and physical activity variables did not predict variation in [Formula: see text] once sex had been accounted for. The highly active and energy-demanding lifestyle of rural Kenyan adolescents may account for their exceptional aerobic fitness and collectively prime them for later training and athletic success

    Attempts to Image the Early Inflammatory Response during Infection with the Lymphatic Filarial Nematode Brugia pahangi in a Mouse Model

    Helminth parasites remain a major constraint upon human health and well-being in many parts of the world. Treatment of these infections relies upon a very small number of therapeutics, most of which were originally developed for use in animal health. A lack of high throughput screening systems, together with limitations of available animal models, has restricted the development of novel chemotherapeutics. This is particularly so for filarial nematodes, which are long-lived parasites with a complex cycle of development. In this paper, we describe attempts to visualise the immune response elicited by filarial parasites in infected mice using a non-invasive bioluminescence imaging reagent, luminol, our aim being to determine whether such a model could be developed to discriminate between live and dead worms for in vivo compound screening. We show that while imaging can detect the immune response elicited by early stages of infection with L3, it was unable to detect the presence of adult worms or, indeed, later stages of infection with L3, despite the presence of worms within the lymphatic system of infected animals. In the future, more specific reagents that detect secreted products of adult worms may be required for developing screens based upon live imaging of infected animals

    Effect of 28 days of creatine ingestion on muscle metabolism and performance of a simulated cycling road race

    <p>Abstract</p> <p>Purpose</p> <p>The effects of creatine supplementation on muscle metabolism and exercise performance during a simulated endurance road race was investigated.</p> <p>Methods</p> <p>Twelve adult male (27.3 ± 1.0 yr, 178.6 ± 1.4 cm, 78.0 ± 2.5 kg, 8.9 ± 1.1 %fat) endurance-trained (53.3 ± 2.0 ml* kg<sup>-1</sup>* min<sup>-1</sup>, cycling ~160 km/wk) cyclists completed a simulated road race on a cycle ergometer (Lode), consisting of a two-hour cycling bout at 60% of peak aerobic capacity (VO<sub>2peak</sub>) with three 10-second sprints performed at 110% VO<sub>2 peak </sub>every 15 minutes. Cyclists completed the 2-hr cycling bout before and after dietary creatine monohydrate or placebo supplementation (3 g/day for 28 days). Muscle biopsies were taken at rest and five minutes before the end of the two-hour ride.</p> <p>Results</p> <p>There was a 24.5 ± 10.0% increase in resting muscle total creatine and 38.4 ± 23.9% increase in muscle creatine phosphate in the creatine group (<it>P </it>< 0.05). Plasma glucose, blood lactate, and respiratory exchange ratio during the 2-hour ride, as well as VO<sub>2 peak</sub>, were not affected by creatine supplementation. Submaximal oxygen consumption near the end of the two-hour ride was decreased by approximately 10% by creatine supplementation (P < 0.05). Changes in plasma volume from pre- to post-supplementation were significantly greater in the creatine group (<sup>+</sup>14.0 ± 6.3%) than the placebo group (<sup>-</sup>10.4 ± 4.4%; <it>P </it>< 0.05) at 90 minutes of exercise. The time of the final sprint to exhaustion at the end of the 2-hour cycling bout was not affected by creatine supplementation (creatine pre, 64.4 ± 13.5s; creatine post, 88.8 ± 24.6s; placebo pre, 69.0 ± 24.8s; placebo post 92.8 ± 31.2s: creatine vs. placebo not significant). Power output for the final sprint was increased by ~33% in both groups (creatine vs. placebo not significant).</p> <p>Conclusions</p> <p>It can be concluded that although creatine supplementation may increase resting muscle total creatine, muscle creatine phosphate, and plasma volume, and may lead to a reduction in oxygen consumption during submaximal exercise, creatine supplementation does not improve sprint performance at the end of endurance cycling exercise.</p

    Nutrition Strategies for Triathlon

    Contemporary sports nutrition guidelines recommend that each athlete develop a personalised, periodised and practical approach to eating that allows him or her to train hard, recover and adapt optimally, stay free of illness and injury and compete at their best at peak races. Competitive triathletes undertake a heavy training programme to prepare for three different sports while undertaking races varying in duration from 20 min to 10 h. The everyday diet should be adequate in energy availability, provide CHO in varying amounts and timing around workouts according to the benefits of training with low or high CHO availability and spread high-quality protein over the day to maximise the adaptive response to each session. Race nutrition requires a targeted and well-practised plan that maintains fuel and hydration goals over the duration of the specific event, according to the opportunities provided by the race and other challenges, such as a hot environment. Supplements and sports foods can make a small contribution to a sports nutrition plan, when medical supplements are used under supervision to prevent/treat nutrient deficiencies (e.g. iron or vitamin D) or when sports foods provide a convenient source of nutrients when it is impractical to eat whole foods. Finally, a few evidence-based performance supplements may contribute to optimal race performance when used according to best practice protocols to suit the triathlete’s goals and individual responsiveness
